Weird black box outline around selections throughout OS X!?

On one of the Macs in our lab (G5 iMac running OS X 10.4.6), someone somehow did something that resulted in there being a weird black box outline around selections throughout the OS (e.g., Finder, Safari). For example, the selected icon on the desktop will now have a black box around it in addition to being highlighted (screenshot below).

I have a VAGUE memory of seeing this once before (and maybe there was some key sequence to activate it?), but can't figure out what the heck it is or how to turn it on/off! I've checked Keyboard preferences, Universal Access prefs, Finder prefs, view options...

Anyone know what the heck?
