I submitted this hint a while back but it hasn't made it to the main page yet.

One can make their own free ringtones with iPhone 1.1.2 by adding 30 to 40 second aac files to iTunes. The ringtone file suffix must be changed to .m4r before importing it to iTunes.

More info here:
http://www.tuaw.com/2007/11/11/itunes-7-5-and-iphone-1-1-2-return-custom-ringtones/

If you use iTunes to convert a short selection to the ringtone, you'll have to delete the original conversion from the iTunes library and import the new one into iTunes, which will put it in the 'Ringtones' section of your library and allow it to be sync'ed to the iPhone. iTunes will accept ringtones longer than 40 seconds, but they won't show up in the iPhone ringtones section of the iPhone tabs when your phone is connected and they won't sync over to the phone (but it will show up in the ringtone section of the library). None of my ringtones ever play to the end because it goes to voicemail before the 30 seconds is over...I suppose a short ringtone would get 'cycled' until the call is sent to voicemail.

I've made several ringtones by copying a song directly from the original cd into garageband, then cutting it down to size and adjusting the volume as needed (I like to fade out at the end instead of an abrupt end). I then exported the file to the desktop, changed the suffix from m4a to m4r and double-clicked to import to iTunes.

I use Quicktime Pro to trim songs. I export them as .m4a and then rename to .m4r.

I make ringtones for my iphone similar to you, but instead of exporting the file, changing the file extension, and then importing into iTunes, I use the "Send Ringtone to iTunes" option in the "Share" drop-down menu of GarageBand 4.1.1. This is the simplest method of making custom ringtones I have used.

I can no longer sync ringtones to iTunes. The Type shows up as Ringtone, but they are in the Music section, not the Ringtones section and they don't sync to the iPhone.

As a workaround, i can use iToner to get ringtones on the iPhone, but this doesn't solve the problem.

1) Has this problem occurred since 7.6 or 7.6.1?

2) Is there a maximum length, bitrate, frequency, etc that must be adhered to for the the Ringtones to show up correctly?

Well, I bought an iPhone yesterday (v. 1.1.4). I used solipsism's advice.

The track in question (snipped by me from a movie and converted to .m4a) resides in iTunes 7.6, which I left closed for most of these steps. I navigated to the track in the Finder, and used QuickTime Pro 7.4.1 to open it. I trimmed it a tiny bit, and exported the result as .mov back into the iTunes Music directory where its parent is. I changed the extension to .m4r, shrugging aside the Finder's protest. I dropped the renamed .m4r track onto the iTunes 7.6 icon in my Dock (to import it), I enabled syncing of ring tones for the iPhone, I synced the phone, and now it's my ring tone. The Mac is OS 10.4.11.

This is a long windy version of what solipsism says, but it seems people are having trouble doing this, and perhaps the extra detail will help someone, perhaps including solipsism, after reading the second post by that author.

Try the MakeiPhoneRingTone from here. (http://rogueamoeba.com/freebies/) Despite what it says it also works with iTunes 7.6 and iPhone1.1.4.
